As a new patient, please be aware of the following requirements and information for your upcoming visit to All Health Medical:

Referral: You must have a referral from a doctor to schedule an appointment with our pain management specialist.

Encounter Notes: Please provide the last three encounter notes regarding your pain problem.

Imaging and Lab Results: Bring all related MRI, X-ray, CT, and lab results associated with your pain issue.

Specialist Information: Include the names and last encounter notes from any specialists you have seen concerning your pain issue, such as:

• Rheumatology
• Spine Specialist
• Neurosurgery
• PM&R
• Auto Accidents
• Orthopedics
• Neurology
• GI
• Kidney, etc.

 

Additional Important Information:

• Referral Requirement: You must have a referral from a doctor to schedule an appointment.
• Documentation: Ensure all required documents are bundled and ready by the time of your first appointment.
• Medication Requests: Providing all the necessary documents does not guarantee that any particular type of medication request will be filled, honored, or guaranteed; this is at the doctor’s discretion.
• Medication Fulfillment: Medications may or may not be filled or refilled at the first appointment.
• Appointment Duration: The first appointment may last up to one hour. Please arrive 15 minutes prior to your appointment to complete the necessary paperwork.
